Haskell L, Tavender EJ, Wilson C, O’Brien S, Borland ML, Cotterell L, Babl F, Sheridan N, Oakley E, Dalziel SR, for PREDICT. “Improving the management of infants with bronchiolitis: developing tailored research translation interventions for a cluster randomised controlled trial“, The 7th Annual NHMRC Symposium on Research Translation, 27-28 November 2018, University of Sydney. More information
Aims: To present a theory driven approach to the design of tailored research translation interventions in the paediatric acute care setting.
Methods: Interventions were developed using a 3 stepped approach: 1) qualitative interviews were undertaken with emergency and paediatric inpatient clinicians in Australia and New Zealand using the Theoretical Domains Framework to identify factors influencing bronchiolitis management; 2) mapping of behavior change techniques to identified key influencing factors and 3) development of research translation interventions based on evidence of effectiveness of change strategies and feasibility.
Results: We will discuss the principles and realities of intervention development. This intervention is currently being evaluated as part of a large Australasian cRCT in 26 hospitals including emergency departments and paediatric inpatient areas.
Conclusion: Results will add to the body of evidence around development and effectiveness of tailored interventions in general, and the paediatric acute care setting specifically.

More information
Aims: A high-quality, evidence based, and consensus led guideline was developed for the management of bronchiolitis in infants presenting to or admitted into hospital, in order to align and potentially improve consistency of Australasian practice and the health outcomes for these infants.
Methods: The guideline was developed using a process adapted from the nine steps recommended by the National Health and Medical Research Council (NHMRC) and the Grading of Recommendations Assessment, Development and Evaluation methodology (GRADE) methodology. Formulation and drafting of the Guideline included identification and drafting of PICOt questions, a systematic literature search and, review and grading of evidence by the NHMRC process and GRADE methodology. Consensus with the Guideline Development Group was sought using nominal group technique (NGT) principles to formulate the clinical practice recommendations and practice points. Key paediatric health professional bodies were consulted and reviewed the Guideline to ensure it was relevant to management of bronchiolitis in the Australasian emergency and ward setting.
Results: The final guideline consists of a useable clinical interface for bed-side functionality (including flow diagrams and tables of key points for example) with a descriptive summary of the evidence base and evidence tables (NHMRC and GRADE) appendices for each key statement.
Conclusion: The development of this evidence-based clinical guideline was a complex process. The involvement of stakeholders who are ultimately the end users, provided valuable clinical knowledge however did present challenges, with committee members located over 2 countries and 5 time zones and with variable baseline knowledge of evidence grading.
